Income Sliding Scale Eligibility

We offer a limited number of sliding scale appointments to support clients who are uninsured, underinsured, or experiencing financial hardship.

• Sliding scale availability is limited and offered on a first-come, first-served basis.

• Sliding scale services are provided as self-pay only and are not billed to insurance.

• Reduced fees cannot be combined with insurance billing or other discounts.

• Eligibility is based on self-reported income,  income documentation may be requested and reviewed periodically.

• Payment is due at the time of service. Clients receiving sliding scale services are subject to the same cancellation and attendance policies as all clients.

• Appointments canceled with less than 24 hours’ notice or missed without notice may result in a no-show or late cancellation fee and loss of sliding scale eligibility.
