At Elevate Health and Nutrition, I work collaboratively with healthcare providers to deliver comprehensive, evidence-based nutrition and diabetes care that supports your patients’ treatment plans and health goals.

 

As a Registered Dietitian Nutritionist (RDN) and Certified Diabetes Care and Education Specialist (CDCES), I provide individualized Medical Nutrition Therapy (MNT) and Diabetes Self-Management Education and Support (DSMES) for adults, children, and families. My focus is on empowering patients to take an active role in their health through practical, sustainable behavior change. I believe in building strong partnerships with medical providers to extend the reach of care and help patients achieve lasting health improvements through nutrition, education, and support.

• Type 1, Type 2, and gestational diabetes

  • CGM placement/removal, setup and education, CGM data monitoring with patient​

  • Glucose meter education, blood sugar monitoring frequency and education

  • Insulin use (all types, first time use, troubleshooting, safety, when and how to use etc.)

  • GLP-1 and other diabetes medication use, education and adherence

  • Medication adherence related to heart health and diabetes

  • Weekly or regular glucose log monitoring for gestational diabetes

  • Preventing long term complications

  • Managing hyperglycemia/hypoglycemia

  • Healthy eating, exercise, healthy coping

• Prediabetes and insulin resistance

• Hypertension, hyperlipidemia, and heart disease

• Renal and liver conditions

• Pregnancy-related nutrition care and weight management

• Food allergies and intolerances

• General healthy eating, malnutrition, or weight concerns

  • On GLP-1 and other weight loss medication: building and maintaining a healthy lifestyle, treating or preventing malnourishment or other complications from undernourishment from weight loss meds​

 

At Elevate Health and Nutrition, we strive to make high-quality nutrition and diabetes care accessible. We are now accepting most major private insurances and Medicare for the state of Colorado. Accepting some insurance carriers in Idaho.​

 

Unfortunately, at this time we can only accept self-pay for the states of (AZ, CA, CT, GA, IA, KY, MI, MO, NC, OR, TX, VA, WI) through HSA/FSA or debit/credit cards.

​How to Refer

Please complete the secure referral form (e-mailed upon request) or fax patient information to 303-845-6829. Patient referral form is not required as long as the following information is included in the faxed information:

• Date of referral

• Patient details: Full name, date of birth, address, phone #

• Insurance information (card copy is sufficient): Name, policy #, Group #, phone #)

• Provider name, NPI if available, practice name, fax and phone #

• Required: diagnosis code(s) related to referral and ICD-10 codes

• Other helpful but optional information: pertinent health history, labs, exercise restrictions, preferred language if other than English 

 

 

Once received, we will confirm visits covered by insurance and contact the patient directly to schedule an appointment. We will provide follow-up communication and progress updates as requested. To request a provider referral form please contact us via email or telephone.
