At Elevate Health and Nutrition, I work collaboratively with healthcare providers to deliver comprehensive, evidence-based nutrition and diabetes care that supports your patients’ treatment plans and health goals.

 

As a Registered Dietitian Nutritionist (RDN) and Certified Diabetes Care and Education Specialist (CDCES), I provide individualized Medical Nutrition Therapy (MNT) and Diabetes Self-Management Education and Support (DSMES) for adults, children, and families. My focus is on empowering patients to take an active role in their health through practical, sustainable behavior change. I believe in building strong partnerships with medical providers to extend the reach of care and help patients achieve lasting health improvements through nutrition, education, and support.

At Elevate Health and Nutrition, we strive to make high-quality nutrition and diabetes care accessible. We are actively working on accepting major private insurances and medicare as forms of payment and will post updates here and in our newsletter, sign up here.

Please complete the secure referral form. Once received, I will contact the patient directly to schedule an appointment and will provide follow-up communication and progress updates as requested. To request a provider referral form please contact us via email or telephone.
